FUNDS DISBURSEMENT ANNOUNCEMENT
The 2012 deadline to apply for a Funds Disbursement from the Rotary Club of Five Points has been extended to May 31, 2012.
If you are interested in applying for a contribution to your non-profit organization, please submit the Funds Disbursement Form along with a Letter of Request on your organization’s letterhead and a current Financial Report for your organization. All items must be received by May 31, 2012 to be considered. Our mailing address is Rotary Club of Five Points, PO Box 5472, Columbia, SC 29250. You may also email pdf copies of the required documents to 5pointsinfo@5pointsrotary.org. Please place in the subject line FUNDS DISBURSEMENT REQUEST 2012. |
